Temporary residence — work — which documents you need

The document list PL360• prepares for “Temporary residence — work” — the same as in the app: 30 items (mandatory: 13, depending on your situation: 15, often requested by the office: 2).

The list is for the Lower Silesian office (DUW). Other offices may ask for more — we will tell you at the consultation.

Always needed

• Passport — scans of all pages (Paszport) — we do it at the office
• Photo 35×45 mm (Zdjęcie) — we do it at the office
• Proof of stamp duty payment (Dowód opłaty skarbowej) — we do it at the office
• Annex 1 from the employer — form depends on when the case started: until 26.04.2026 old, from 27.04.2026 new (MOS) (Załącznik nr 1) — we do it at the office
• Social insurance registration: ZUS ZUA or ZUS ZZA (Zgłoszenie do ubezpieczeń)
• Employer’s tax no-arrears certificate (US), not older than 30 days (Zaświadczenie o niezaleganiu w podatkach) — needed for the fingerprints appointment; must not be older than 30 days on the submission date
• Employer’s ZUS contributions no-arrears certificate, not older than 30 days (Zaświadczenie o niezaleganiu w składkach) — needed for the fingerprints appointment; must not be older than 30 days on the submission date
• Current residence card — both sides (Karta pobytu) — only if your basis is: Residence card
• Current decision granting the residence permit (Decyzja o zezwoleniu na pobyt) — only if your basis is: Residence card
• Proof of UKR status (PESEL with UKR status or Diia.pl document) — only if your basis is: UKR status (PESEL UKR)
• Signed employment or mandate contract (Umowa o pracę / zlecenia) — optional
• Annual PIT-37 tax return with UPO for the last year before applying — not the employer’s PIT-11 (PIT-37 + UPO) — optional
• Tax office certificate of no arrears (Zaświadczenie o niezaleganiu z US) — optional

Depending on your situation

• Power of attorney + 17 zł fee (Pełnomocnictwo)
• Family member’s income certificate — if you have no employment contract (umowa o pracę): zlecenie, dzieło, none (Dochód członka rodziny)
• Power of attorney to sign Annex 1 when the signatory is not an authorised representative (Pełnomocnictwo do Załącznika nr 1)
• If the employer has arrears: documents on the amount, period and exemption / deferral / instalments (Dokumenty o zaległościach)
• Professional qualification documents — regulated professions (Kwalifikacje zawodowe) — A sworn translation into Polish is required
• Driver: certified copy of the driving licence; taxi — Polish licence (Prawo jazdy)
• Driver: medical and psychological examinations (Badania lekarskie i psychologiczne)
• Truck driver over 3.5 t: driver attestation (Świadectwo kierowcy)
• Truck driver over 3.5 t: certificate of professional competence, code 95 (Świadectwo kwalifikacji zawodowej)
• Taxi: employer’s licence for domestic passenger transport by taxi (Licencja na przewóz osób taksówką)
• Criminal record certificate (Zaświadczenie o niekaralności)
• Temporary work agency: user employer’s statement since when you work for them (Oświadczenie pracodawcy użytkownika)
• Temporary work agency: statement for which company and under whose direction you work (Oświadczenie podmiotu powierzającego pracę)
• If Załącznik nr 1 names a user employer: its tax and ZUS no-arrears certificates, not older than 30 days (Zaświadczenia pracodawcy użytkownika) — must not be older than 30 days on the submission date
• Company documents — board member, commercial proxy (Dokumenty spółki)

Often requested by the office

• Health insurance — ZUS or a policy (Ubezpieczenie zdrowotne)
• Statement of address for the residence card (Oświadczenie o adresie)
