- Proof of stamp duty payment (Dowód opłaty skarbowej) — we do it at the office
- Current residence card — both sides (Karta pobytu) — only if your basis is: Residence card
- Current decision granting the residence permit (Decyzja o zezwoleniu na pobyt) — only if your basis is: Residence card
- Proof of UKR status (PESEL with UKR status or Diia.pl document) — only if your basis is: UKR status (PESEL UKR)
- Certificate from the school or university that you are a student (Zaświadczenie ze szkoły / uczelni)
- Class schedule for the whole period of study (Plan zajęć)
- Proof of tuition payment or fee exemption (Opłata za naukę lub zwolnienie)
- Tenancy agreement — valid for at least 3 more months (Umowa najmu)
- Health insurance for at least one year (Ubezpieczenie zdrowotne)
- Funds for living and return: 2,500 zł for return + living costs for each month of study + rent (Środki finansowe) — must not be older than 31 days on the submission date
- Statement of living costs (Oświadczenie o kosztach utrzymania)
- Statement on the number of dependants (Oświadczenie o liczbie osób na utrzymaniu)
