- Current decision granting the residence permit (Decyzja o zezwoleniu na pobyt) — only if your basis is: Residence card
- Proof of UKR status (PESEL with UKR status or Diia.pl document) — only if your basis is: UKR status (PESEL UKR)
- Tax office certificate of no arrears (Zaświadczenie o niezaleganiu z US)
- ZUS contributions no-arrears certificate (Zaświadczenie o niezaleganiu w składkach)
- Criminal record certificate (Zaświadczenie o niekaralności)
- Proof of business activity: invoices, contracts, income and expense ledger, PIT returns (Potwierdzenie działalności)
- Business plan for 24 months (Biznesplan) — not needed if the company has been operating for more than 12 months
- Tenancy agreement — valid for at least 3 more months (Umowa najmu)
- Statement on the number of dependants (Oświadczenie o liczbie osób na utrzymaniu)
- Social insurance registration: ZUS ZUA or ZUS ZZA (Zgłoszenie do ubezpieczeń)
